SUMMARY:
This position provides a full range of direct case management to Transitional Aged Youth (TAY) clients receiving Urgent Accommodation Vouchers for 14-day hotel stays. This position evaluates client needs and develops a service plan with the main goal of a shelter placement for approximately 20 active clients by the end of their 14-day hotel stay. The goal is to transition the TAY individual from the Urgent Accommodation Voucher Program to a shelter placement as quickly as possible. This is a Full-time, non-exempt, union position.

The UAV TAY Case Manager position starts at $30.00 per hour.

DESCRIPTION OF PROGRAM SERVICES:
The Urgent Accommodation Voucher Program is a low barrier model that creates a safety net for households to quickly access an adequate and safe place to stay for a limited time. The program provides for emergency hotel placement operations and supportive services, including linkages to existing programs such as Coordinated Entry, harm reduction services, public program benefits, health services, and behavioral health.
RESPONSIBILITIES:
- Provide direct client services: individual peer counseling, case management, advocacy, and accompaniment.
- Conduct new client intake within 24 hours of the first day of their hotel stay
- Client information must be entered into the ONE database within 48 hours.
- Ensure client meets with Coordinated Entry within 72 hours of intake and monitor client activity related to achieving personal goals.
- Create a service plan for each client with key issues prioritized.
- Partner with service providers as needed to meet each client’s service plan goals by coordinating care.
- Assist clients in maintaining stability by offering support and linkage to services.
- Problem solves with the client to determine how best to assist the client.
- Facilitate meetings and support groups as needed.
- Develop client exit survey and case summary to support outcomes and program analysis.
- Maintain electronic client records in Salesforce, individual case files, daily logs, and service statistics in an accurate and timely manner.
- Participate in regular staff meetings including case management meetings.
- Perform other appropriate duties as assigned.
QUALIFICATIONS:
- BA degree in social services, psychology, or another related field. Equivalent work experience may be accepted in lieu of a degree.
- Two years’ experience providing services to chronically homeless preferred. Relevant comparable experience will be considered.
- Above average proficiency to read, write and speak English.
- Demonstrated skills in counseling, case management, and client advocacy.
- Familiarity with the philosophy and approach of Harm Reduction, Motivational Interviewing and OARS.
- Strong interpersonal, verbal, and written communication skills.
- Understanding of homeless issues, especially in context to race, class, gender, and sexual identity.
- Understanding of coordinated entry system.
- Knowledge, experience, and comfort working with a diverse community, including LGBTQQ, youth, immigrant, low income communities, and communities of color.
- Proficient in using MS Office including Word, Outlook, and Excel.
- Working knowledge of community resources and familiarity with SF homeless shelter systems and services.
- Demonstrated commitment to open, direct, and non-violent communication.
- Ability to de-escalate crisis situations and make decisions under pressure.
- Proficient reporting skills including data entry, Excel, and Word processing.
- Strong initiative and ability to work both independently and cooperatively with staff and management.
- Ability to maintain professional boundaries with clients, volunteers, and staff.
